As a chiropractic physician, we can be an attending physician for up to 60 consecutive calendar days or 18 visits (whichever occurs first) and authorize time off work for up to 30 calendar days from the first day of your initial appointment. This means you do not need a referral before visiting our clinic for your work injury.

You should be aware that you are required to fill out an Oregon Form 801, Worker’s and Employer’s Report of Occupational Injury or Disease/Illness, if you want to file a workers’ compensation claim.
